HarmonyCares Palliative Care provides care to people living with complex progressive medical illnesses. We focus on quality of life and the relief of symptoms caused by advanced illnesses. The focus of the Palliative Care team is on the individual and their familyโ€™s quality of life and relief from burdensome symptoms like pain and stress. Our providers work to keep people at home and out of the hospital.

The specialty palliative care team at HarmonyCares is here to provide an extra layer of medical support. Our specialty trained palliative care professionals provide education and assistance in the management of complex conditions. Our primary goal is to help people remain fully independent and at home.

A specialty trained provider will help to navigate complex clinical decisions. Based on the unique clinical needs of the individual, we coordinate with primary care providers and facilitate collaboration between all of the other specialists who may be involved in the care plan.

Our services are a valuable add-on to the other medical care that a person currently receives. Our main goal is to improve patient outcomes and quality of life.
