Veteran Hospice Care Program

Each year, many military veterans require specialized hospice care. Chronic pain,
post-traumatic stress disorder, depression, presumptive diseases, and other
physical or mental illnesses are just some of the conditions that veterans suffer
from that might need extra attention in hospice programs.

We believe only the highest quality health care will do for those who have sacrificed
so much for this country. That’s why HarmonyCares provides medical care and
support that reflects the significant contributions made by these men and women.
Our staff is here to ensure comfort and a high quality of life for veteran patients
and their families.

How We Care for Veterans

HarmonyCares locations have become national partners of the We Honor Veterans program,
a pioneering campaign developed by the National Hospice and Palliative Care Organization
in collaboration with the Department of Veterans Affairs.

As We Honor Veterans Partners, HarmonyCares will implement ongoing veteran-centered
education for our staff and volunteers to help improve the care we provide to our veteran
patients, including providing additional support and communication to coordinate with the
VA for medical services.

A better quality of life for people is our primary goal. HarmonyCares’ Veterans Hospice Care
Program offers several health and wellness benefits to properly assist loved ones through the
finals stages of life. These include emotional support and counseling for veterans to help them
cope with untreatable conditions and the final stages of their lives. We also provide counseling
support for families to understand their loved one’s condition. Finally, many of our volunteers
are veterans and understand the unique experience of our veteran patients, having served in
the armed forces.

By recognizing our nation’s veterans’ unique needs while in hospice, HarmonyCares is better
able to accompany and guide veterans along life’s journey.

We partner with leading insurance companies, offering no-cost health assessments to plan members. Once a health plan identifies people who might benefit from this assessment, we will contact the member to schedule an in-home visit performed by a licensed clinician.
