How We Achieve Advance Care Planning

Advance Care Planning allows individuals to express wishes around care. We
provide healthcare which aligns with the individuals’ values and preferences,
even when those decisions can no longer be made. It also provides peace of mind
to people in our care and their loved ones, knowing that decisions about future
healthcare are made thoughtfully and in advance.

Our team of experience healthcare professionals works closely with families to
create a care plan which honors the unique wishes of their loved one while
providing the highest level of comfort and quality of life. With services like pain
and symptom management, emotional and spiritual support, and advance
directive assistance, HarmonyCares can help make the end-of-life journey as
meaningful and peaceful as possible.
