We offer both in-person and virtual visits

Our main goal in partnering with health plans is to keep you healthy and to keep you in your home. By providing in-home health visits at no cost to you we are able to accomplish our aim and help you carve a path towards better wellness.*

During this 30-45 minute health review, our provider will:

Check your vitals
Review your medications
Discuss your medical history
Provide medical advice and tips to
better understand your health
Give you a checklist of what to ask your family doctor

A visit in the comfort of your home

Whether in-person or virtually, your visit includes one-on-one time with a highly skilled provider.

By working as a team with your family doctor and your health plan, we can help you enjoy happier, healthier days at home.

The visit does not replace medical care that you receive through routine office visits, and is not the same as your yearly check-up with your doctor.
