Matthew Chance

Chief Executive Officer
Get to Know Matt

With over 20 years of experience and a career dedicated to improving home-based healthcare, Matt has been actively helping healthcare organizations solve their most challenging problems. In May 2022, Matt became CEO of HarmonyCares (then U.S. Medical Management) and has been part of further defining the standard for home-based care for the most vulnerable patients who need it most across the country. Prior to joining HarmonyCares, Matt was the CEO of Ready Responders and President of myNEXUS, where he worked to innovate in-home care models which improve health equity and access to care. Before his tenure with Ready Responders and myNEXUS, Matt provided enterprise-wide strategic and operational oversight for all health engagement initiatives for Cigna-Healthspring, including STARS/quality, risk adjustment, customer analytics and engagement. Matt worked with HealthSpring for 9 years and was the COO of HealthSpring of Alabama before moving to Cigna-HealthSpring enterprise operations. While in Alabama, he helped develop various aligned models within the delivery system to better connect the health plan and provider community. Matt began his career in healthcare, working with hospitals and insurers at AIM Healthcare Services. He led operations for the southeast region and was with AIM for 5 years.

Michael Millie, MD

Chief Medical Officer
Get to Know Mike

Mike has a proven track record of achieving financial success, strategic growth, and quality improvement while leading a large portion of our growing business. One of the most important strategies in these successes was pursuing increased engagement and investment in physician leaders at a time when physician leadership budgets were being gutted. The return on this investment was surprising, even to him, and confirmed the theory that providers still have meaningful control in ambulatory medicine.

Mike started by driving small projects within his surgery department and quickly moved to leadership roles in their professional liability insurance group. In 2011, he became chair of their large general surgery department. In 2013, he was promoted to surgery chief for the surgery section and ambulatory surgery centers. Together, with his physician leader colleagues, they have achieved operational excellence, including safety and quality, strategic growth and continuously improving financial performance. Through all of this work, Mike has followed the same principles: honor the patient and respect the provider. Slowly, we are aligning incentives, improving healthcare delivery and, ultimately, improving the nation’s health.

Jennifer Gilchrist

Jenn has been in the healthcare industry for 25 years, spending the last decade concentrating in the value-based care space. Jenn has extensive experience leading clinical operations teams in start-ups and transformational organizations. This role brings her back to a space she knows well and has been passionate about for years. It is important to her that HarmonyCares is successful in driving quality healthcare services into the home for people who need it most.

Heidi Johnson

Chief People Officer
Get to Know Heidi

Heidi is a human resource leader with more than 20 years of experience in development and execution of strategic People and Culture initiatives. She has extensive experience cultivating teams and culture, attracting and developing talent, and developing leadership and performance management programs.

Prior to Harmony Cares, Heidi led the HR function for Crossover Health a $200M primary care company through tremendous growth, more than doubling in company size in her five years. There, she devised and executed on talent strategy and built people and culture processes and programs to ensure success in their growth. 

During the decade prior to joining Crossover, Heidi led HR teams in Talent Acquisition, Talent Management, and HR Business Partner roles for Medtronic across 20 countries, most notably leading projects that transformed HR programs and developed world class solutions across the globe.

Jonathan D'Souza

Chief Product & Technology Officer
Get to Know Jonathan

Jonathan has spent more than 30 years in healthcare (across Epic, Microsoft, and Advisory Board/Optum), primarily focused on building and delivering workflow and decision support solutions for clinical operations and population health.

Prior to joining HarmonyCares, Jonathan served as chief product and technology officer at MedArrive, where he was responsible for the analytics and technology systems that supported MedArrive’s home-based care programs.

Jonathan brings his wealth of knowledge to deliver innovative solution for care in the home to drive our strategic initiatives forward at HarmonyCares.

Zack Mulligan

As chief financial officer, Zack is responsible for overseeing and managing the overall financial performance of the business and for helping set strategic financial priorities. Prior to joining HarmonyCares, Zack was the chief financial officer at naviHealth, a post-acute care coordination business that partners with health plans, health systems, and risk-bearing physician groups. Zack also previously worked in growth equity investing at Pamlico Capital and investment banking at Wells Fargo Securities.

Alexander Sozdatelev

Get to Know Alex

As chief business officer, Alex Sozdatelev leads the charge of business development, growth, strategy, and our Accountable Care Organizations. Prior to joining HarmonyCares, Alex was a Partner at Rubicon Founders, a healthcare services focused investment firm where he led deals in primary care, specialty care, and home-based care. He has spent over ten years at McKinsey & Co, most recently as a Partner at Healthcare practice serving health plans, PBMs, retail pharmacies, PE funds, and PE-backed healthcare companies. Alex holds an MBA from University of Chicago Booth School of Business.

“I grew up in Russia where it is a norm, not an exception, that physicians and nurses are coming to homes to see patients who need care: from children to seniors. I know firsthand that it is a better model to treat patients, especially older and frail. I was genuinely surprised when I first learned that in the US sick patients need to come to a clinic or Emergency Room to get care. So, when I learned about home-based model like HarmonyCares’, I immediately recognized its benefits and got excited to grow such model and make it available to more patients across the country.”

Daniel Santmyer

Chief Actuary Officer
Get to Know Daniel

Daniel brings more than 30 years of experience working as an actuary and risk advisor to healthcare payers and providers. Prior to joining HarmonyCares, he served in diverse roles as a health plan actuary and consulting actuary, as well as previously being Chief Actuary for a value-based care software company, where he assisted healthcare providers and payers with actuarial support and analysis of network value, operational effectiveness, outcomes analysis and value-based care contract modeling and analysis.

Dan has also served in Chief Actuary roles at both Beacon Health Options, a behavioral health management and wellness company, and the National Accounts business division of Anthem/WellPoint.

He is a member of the American Academy of Actuaries and a Fellow of the Society of Actuaries.

Shannon Adams

Shannon has over 20 years of expertise in driving retention, marketing, analytics, and user-centered design. She has led Experience programs at large institutions such as Cigna, HealthSpring, and Vanderbilt University Medical Center, specializing in Net Promoter Score (NPS), CAHPS, and elevating the voice of the customer. Shannon is known for turning insights into action – boosting retention, increasing revenue, and improving customer satisfaction through data-driven innovation and cross-functional collaboration.

I joined HarmonyCares because I believe everyone deserves access to compassionate, high-quality healthcare – especially people with Medicare who often face incredible barriers to care. The mission to deliver care in their home spoke deeply to me, both professionally and personally. Brooks Newman

Chief Legal Officer
Get to Know Brooks

Brooks brings more than 20 years of egal experience in the healthcare industry, most recently as Chief Legal Officer, Chief Compliance Officer, and Corporate Secretary of Confluent Health, a private equity-sponsored national family of musculoskeletal companies.

As a member of the Executive Leadership Team at Confluent, Brooks was a principal advisor on corporate strategy and innovation, mergers and acquisitions, corporate governance, private equity sponsorship, and corporate finance.

Prior to Confluent Health, Brooks was SVP, Deputy General Counsel, and Corporate Secretary at Humana Inc., where he served for over 12 years, including as Chief Counsel for the Government Programs and Healthcare Delivery segments.

Brooks has extensive experience and passion for innovative public/private partnerships that drive innovative care models that align care and cost incentives across stakeholders. He was captivated by HarmonyCares’s mission and vision as a leading value-based care provider of in-home primary care services for people with complex healthcare needs.

Scott Vasey

President, Home Health and Hospice

Scott Vasey has worked in health care for over thirty years.  Starting out as a social worker, he has a passion for “communities of caring”, with an emphasis on the quadruple aim of improving quality, reducing overall cost, and improving both member and provider satisfaction.  Over the course of his career he has led payer and provider strategy for such organizations as UPMC, UHC/Optum, VNS Health, myNEXUS/Carelon, and AmeriHealth Caritas, with a focus on improving quality and outcomes, operational performance and technology integration, and lowering total cost, with ten-plus years solely focused on the home and community.  Scott has worked with thousands of organizations in almost every state, and has also been a speaker on value-based care models in post-acute and community-based care.

Purav Bhatt

Chief Transformation Officer
Get to Know Purav

Purav joined Harmony Cares in September 2023 as the Chief Transformation Officer, where he oversees Clinical Product Operations, Lab & Diagnostics, CHA operations, Data analytics and Technology. Prior to joining Harmony Cares, he served as the Chief Operating Officer at Evergreen Health Medical Group (WA state) and has held multiple value based medical group operational leadership roles in Optum.  He has also served on the advisory committee for Healthcare Cost Transparency Board in WA state. With over a decade of experience leading technology enabled clinical operations, he is leading the charge for scaling care delivery infrastructure at Harmony Cares.
